2. Regulated Activity

Example Weight Loss Clinic is registered with the Care Quality Commission (CQC) to carry out the following regulated activity under the Health and Social Care Act 2008:

Treatment of Disease, Disorder or Injury (TDDI)

Including the prescribing and supply of prescription-only medicines (POMs) for the management of obesity and overweight in adults, delivered via remote consultation.

All prescribing is carried out by qualified independent nurse prescribers who hold current NMC registration and are authorised to prescribe from the full British National Formulary (BNF) within their area of competence.

3. Service User Group

The service is provided to adults aged 18 years and over who meet the clinical eligibility criteria for medicated weight management. Eligibility is assessed on an individual basis and includes consideration of:

  • Body Mass Index (BMI) of 30 kg/m² or above; or BMI of 27 kg/m² or above with at least one weight-related comorbidity
  • Absence of contraindications to prescribed treatment
  • Capacity to consent to treatment
  • Ability to engage with the remote consultation model

The service does not accept patients under the age of 18, patients who are pregnant or breastfeeding, or patients with conditions that constitute absolute contraindications to the proposed treatment.

4. Aims and Objectives

The aims of Example Weight Loss Clinic are to:

  • Provide safe, effective, and person-centred medicated weight management services to eligible adults in the United Kingdom
  • Ensure all clinical decisions are made by qualified, NMC-registered independent prescribers in accordance with current NICE guidance, MHRA regulations, and evidence-based best practice
  • Deliver services that are accessible, convenient, and responsive to individual patient needs through a remote consultation model
  • Support patients to achieve clinically meaningful and sustainable weight loss outcomes through a combination of pharmacological treatment and lifestyle guidance
  • Maintain the highest standards of clinical governance, patient safety, and data protection
  • Operate transparently and in full compliance with CQC registration requirements, the NMC Code, and all applicable legislation

5. How the Service is Delivered

Services are delivered entirely via remote consultation using secure video technology (Microsoft Teams). The clinical pathway is as follows:

Online Eligibility Assessment

Prospective patients complete a structured online questionnaire covering medical history, current medications, contraindications, and weight management goals.

Clinical Consultation

Eligible patients are invited to a video consultation with an independent nurse prescriber, who conducts a full clinical assessment and discusses treatment options.

Prescribing Decision

Where clinically appropriate, the prescriber issues a prescription in accordance with the patient's individual clinical profile and the relevant Summary of Product Characteristics (SmPC).

Ongoing Monitoring

Patients are reviewed at regular intervals via follow-up consultations. Progress, tolerability, and clinical response are assessed at each review.

Discharge or Continuation

Treatment is continued, adjusted, or discontinued based on clinical response, patient preference, and ongoing eligibility.

6. Clinical Governance

Example Weight Loss Clinic maintains robust clinical governance arrangements, including:

  • Prescribing oversight: All prescribers operate within their individual scope of practice and are subject to regular clinical supervision and appraisal.
  • Incident reporting: A formal incident reporting and learning system is in place. All significant events are reviewed and acted upon.
  • Complaints management: A structured complaints procedure is in place. All complaints are acknowledged within 3 working days and investigated within 20 working days.
  • Safeguarding: All clinical staff have completed safeguarding training appropriate to their role. The named safeguarding lead is the Registered Manager.
  • Medicines management: Prescribing is conducted in accordance with MHRA guidance, the relevant SmPC, and NICE clinical guidelines.
  • Data protection: All patient data is processed in accordance with UK GDPR and the Data Protection Act 2018. The service is registered with the ICO.

7. Quality and Continuous Improvement

We are committed to continuous improvement in the quality and safety of our services. We collect patient feedback through our CQC-aligned feedback system, monitor clinical outcomes, and review our policies and procedures on a regular basis. Our CQC inspection ratings and any enforcement actions are published on the CQC website and will be communicated to patients as appropriate.
